As our Community Care Engagement Specialist, you'll help bridge the gap between participants and their wider communities, creating opportunities for social connection, skill development, and participation in community life.

Role Purpose

As our Community Care Engagement Specialist within the National Care Department, you'll be an integral part of our team that owns and executes the end-to-end participant experience. Your focus will be on developing and implementing strategies for community integration and social participation for NDIS participants.

You'll identify community resources, develop partnerships with local organizations, and design programs that facilitate meaningful community engagement. Working collaboratively with other members of the National Care team, you'll ensure that social inclusion is a key component of each participant's support plan.

In line with your department's WorkWell™ contribution, you'll enhance the participant experience by connecting them to social inclusion opportunities and community events as needed. You'll also provide valuable feedback on community engagement efforts to drive continuous improvement in our approach to holistic NDIS support.

Key Responsibilities

Community Resource Mapping & Partnership Development

  • Research and map community resources and activities that align with participant interests, goals, and access needs

  • Develop and maintain partnerships with community organizations, recreational facilities, educational institutions, and other local resources

  • Negotiate accessibility accommodations and inclusive opportunities with community partners to ensure successful participant engagement

  • Create and maintain a comprehensive database of community resources that can be matched to participant interests and support needs

As Community Care Engagement Specialist, you play a vital role in our WorkWell™ Model, particularly within:

  • Service Delivery & Participant Experience

    You'll enhance the participant experience by connecting them to social inclusion and community engagement opportunities that complement their formal supports

  • Intake & Needs Mapping

    You'll contribute to participant "Blueprints" by adding community engagement plans and preferences that support holistic NDIS goals

  • Performance Monitoring & Feedback

    You'll gather critical feedback about community engagement successes and challenges to inform continuous improvement
