NDIS Support Coordinator

As an NDIS Support Coordinator, you'll be the navigator and advocate that empowers participants to make the most of their NDIS funding and access quality services that align with their aspirations, needs, and preferences.

Role Purpose

As an NDIS Support Coordinator within our Operations Department, you'll be part of the Service Delivery team that ensures flawless day-to-day execution across all support services. You'll be the critical link between our participants and the services they need to thrive, helping them understand their NDIS plans, connect with quality providers, and overcome barriers to achieving their goals.

You'll work closely with participants to develop and implement support coordination strategies that maximize their NDIS funding, building their capacity to navigate the system while ensuring they receive consistent, high-quality supports. As part of the Operations team, you'll contribute to maintaining our elite, audit-ready standard of service.

In line with your department's WorkWell™ contribution, you'll execute on-the-ground support (part of the "engine room"), maintain compliance and audit trails, and enable rapid operational response to participant needs. Your role requires a deep understanding of the NDIS ecosystem, strong relationship-building skills, and the ability to advocate effectively.

Key Responsibilities

NDIS Planning & Implementation

  • Work with participants to understand their NDIS plans, goals, and support needs based on their participant Blueprint

  • Develop comprehensive support coordination plans that align available funding with participant goals and priorities

  • Implement and monitor support plans, ensuring services are delivered as agreed and adjusting strategies as participant needs evolve

  • Prepare participants for plan reviews by gathering evidence, documenting outcomes, and identifying future support needs

  • Service Delivery & Participant Experience

    You'll be on the front lines of ensuring services align with participant Blueprints and NDIS plans, coordinating a cohesive support approach

  • Performance Monitoring & Feedback

    You'll gather important feedback on service quality and participant outcomes, helping us improve our support approaches

  • Redeployment & Issue Resolution

    When support adjustments are needed, you'll work with our internal teams to ensure rapid, effective responses to participant needs

Qualifications

  • Experience in NDIS Support Coordination

  • Qualification in Allied Health, Social Work, Community Services, or related field

  • Thorough understanding of NDIS processes and systems

  • Strong advocacy and communication skills

  • Experience working with people with diverse disabilities
